New Patient Information

If you are a new patient to Health Plus General Practice and you are visiting us for the first time you can now fill in your ‘new patient registration form’ here, saving you time before your scheduled appointment.

If you are coming from another practice and would like to transfer your records to our practice. Please fill out a Patient Transfer Into HPGP form and we will send it to your previous practice on your behalf.

Simply complete your forms and either email it to us at contact@healthplusgp.com.au or bring them with you to your appointment.

Patient Transfer Out

Patients who wish to have their medical records transferred to another medical practice are required to provide a signed request form. You can request your gaining practice to request it for you, Signatures are required, or download and complete the form provided on this page. Signatures are required from all family members over the age of 16.
Please specify if you would like a medical summary or the full medical record transferred.

A medical summary is provided free of charge.

Transferring the full medical record in hardcopy or electronic copy will attract an administrative fee of $50.00. All medical records are sent via registered mail.

eHealth (PCEHR) Enrollment

Personally Controlled eHealth Record (PCEHR) and other eHealth enrolment forms are available on http://www.ehealth.gov.au

Patient could also enrol online on the same site.
