Even though you may feel this request is a simple task, a face to face appointment allows your GP to engage in preventive and integrated care, to consider any recent changes to your health status and to ensure you get the best possible medical care.

If you are collecting a script or referral, please ensure you have a Photo ID, which is required before collecting any sensitive patient information from reception.
